Frequently Requested Forms
Access commonly requested OCFS forms and documents through the links below, including essential paperwork and resources frequently used by early childhood programs and providers. Please note: Forms, documents, and links are provided for convenience and may be updated, changed, or removed by the Office of Children and Family Services (OCFS) at any time. Please refer to official OCFS resources for the most current versions and requirements.

To initiate the comprehensive background check process for staff new to child care who have not been fingerprinted previously, individuals must follow the instructions on the OCFS-4930 form to schedule a fingerprinting appointment and also submit a completed OCFS-6000 packet to their program’s provider/director.
All existing individuals who have not had a comprehensive background check completed since September 25, 2019, must schedule an appointment for criminal history fingerprinting and submit a 6000 packet to their regulator or program provider/director.
The Division of Child Care Services developed two forms to help record and track training hours. Family and Group Family providers should use form OCFS-4880 – Individual Training Tracking Form and maintain it on file for renewal submission. School-Age Programs and Day Care Centers should use OCFS-4880 for Directors and individual employee tracking, while Directors should complete form OCFS-4879 for all employees at their site. Form OCFS-4879 must be submitted at renewal and maintained on-site. Both forms are included below and can also be ordered through the OCFS warehouse at (518) 473-0971.
